Transition Specialist II (LRS-TPCA)

Qualifications:

  1. Associate's Degree or equivalent college hours and additional training in data management, vocational education, transition, or equivalent
  2. Preferred: previous work with budgets, data management, education, or transition
  3. Minimum of three years paid related work experience
  4. Experience working in a school setting
  5. Knowledge of distinguishing qualities of special education population
  6. Experience with computer processing (Word, Excel, Etc.)
  7. Ability to organize daily tasks; office functions and schedules
  8. Possess a valid Louisiana driver's license and ability to provide own transportation
  9. Ability to work in a flexible, cooperative and professional manner
  10. Project good interpersonal communication skills
  11. Ability to work independently, prioritize tasks and utilize effective time management skills
Reports To: Director of Special Education, Transition Coordinator, Transition Facilitator 

Supervises: None

Job Duties & Responsibilities:
To enable students to derive the fullest possible educational experience from school by implementing transition services for students to reach maximum independence upon completion of their educational career.

  • Completes a self-evaluation annually
  • Develops and implements Individual Growth Plan (IGP) annually
  • Reports job-related injuries within 48 hours of the incidents to the Director of Human Resources. ("Job related injuries" include, but are not limited to, any accident or illness which involves loss of consciousness, restriction of work motion, or medical treatment.)
Terms of Employment: 10 months

Evaluation: 
Performance of this position will be evaluated annually in accordance with Louisiana Department of Education and Natchitoches Parish School Board policy

Performance Responsibilities:
Essential duties and responsibilities include the following. Other duties may be assigned.
  1. Provide transition services to students with disabilities within Natchitoches Parish School System
    1. Job exploration counseling
    2. Work based training programs
    3. Workplace readiness training
  2. Counseling on transition and post secondary training programs and instruction on self-advocacy
