Position Summary
The primary responsibility of the Clinical Liaison is to facilitate a seamless transition for patients discharging from a facility setting to the care to home or an agency setting for post-acute care needs. The Clinical Liaison will respond to referrals and determine the level of care required by the patient upon initial evaluation. Following identification of new LTAC/IRF patient criteria, the Clinical Liaison will complete a full risk-assessment of the patient, including a cost indicator using the net reimbursement worksheet. In addition, the Clinical Liaison will implement re-hospitalization reduction initiatives for patients with Acute Care Hospitalization risk and continually communicate between healthcare providers during all phases of transition from the facility to the next level of care. The Clinical Liaison establishes best practice intervention and education to improve patient outcomes and promote patient self-management upon discharge from the LTAC/IRF to the next level of care. The Clinical Liaison role is clinically focused and aligned with company market development initiatives. This clinical liaison understands their role in growth, serving more patients and delivering exceptional care across the entire post-acute care continuum. Using discretion and independent judgement, the Clinical Liaison identifies specific referral target audiences and develops referral-building strategies in collaboration with the Administrator. The Clinical Liaison will interact with all departments and services as needed to complete the referral evaluation process and address barriers to admission by working effectively with referral sources and the Admissions Team.
Primary Responsibilities
- 1. Responds to patient referrals within an hour of notification of the referral.
- 2. Visits the patient to explain doctor’s orders, services and LECH procedures and obtains all necessary information to facilitate transfer.
- 3. Completes an accurate and comprehensive head to toe functional assessment of patients that are referred to the hospital.
- 4. Verifies address, phone number and payer information, ensures eligibility criteria are met and coordinates the organization and transferring of orders and referral information to the applicable agency/facility staff.
- 5. Reviews patient assessments within 1-2 hours with the Medical Director.
- 6. Notifies the referral source of acceptance or denial of a patient within 2 hours of receiving the referral.
- 7. Required to obtain Administrator and DON final approval on all recommended admits.
- 8. Involves the family/caregivers in the educational process, assesses post-discharge educational/coaching needs.
- 9. Upon admit, coordinates the post-facility discharge follow-up for patients whose anticipated length of stay will be shorter and assists the 11. 10.LECH in preparation of accepting care of the patient.
- 11. Actively participates in daily staff meetings at the facility to ensure patient satisfaction and to validate continuity of care.
- 12. Serves on facility committees, if requested, and works with hospital focus groups to assist in systems integration and process improvements which result in improved patient outcomes and transitions of care.
- 13. Participates in meetings as scheduled with staff, DON, Medical Director and Medical Advisory Board.
- 14. Attends monthly calls and company provided in-service when required.
- 15. Provides educational in-services to effectively communicate the features, benefits and specialty programs of LECH and to educate staff as to what services are available.
- 16. Develops and improves relationships with referring physicians and other entities in the primary target markets.
- 17. Locates and educates new referral sources on the value of our unique services and programs.
- 18. Communicates with growth team and continually analyzes best practices and opportunities to provide care to and reach any underserved population within our service areas.
- 19. Develops and implements an effective census growth plan for achieving a budgeted admit goal.
- Maintains appropriate statistics and generates reports as required by the facility’s policy and procedure.
- 20. Follows-up on internal and external barriers as related to the referral/admission process to ensure rapid solutions.
- 21. Reviews and provides written analysis of denial tracking on a monthly basis to the facility Administrator and recommends a plan of action.
- 22. Effectively, ethically and professionally promotes LECH at conventions, seminars and professional meetings.
- 23. Actively implements new ideas to promote the facility and the services it provides.
- 24. Maintains and promotes good public relations with the community by actively participating in opportunities to promote and market.
- 25. Practices performance management with an ultimate goal of increasing admits.
- 26. Models and fosters open communication among departments, teams and individuals.
- 27. Exhibits diplomacy and team-building skills in accomplishing responsibilities.
- 28. Allocates the appropriate level of resources based on the priorities.
- 29. Demonstrates personal responsibility in regards to time management.
- 30. Attends Referral Development meetings as directed by the Administrator.
- 31. Accepts/incorporates constructive feedback from peers/superiors.
- 32. Maintains confidentiality at all times and is ethical in manner when dealing with physicians, staff, patients and families.
- 33. Completes all required expense paperwork and scheduled events in the Playmaker CRM database in line with company policies.
- Must have computer skills with knowledge of spreadsheet and word processor software.
- Clerical and business machine skills and knowledge of business accounting.
- Good organizational and personal communication skills.
Required Education and Experience
- Current LPN or RN license in state of practice.
- One (1) year acute care hospital experience.
- Must have independent means of transportation, possess a valid driver’s license and the ability to drive.
- Automobile LIABILITY insurance coverage must be current.
Previous Clinical Liaison experience preferred.
